Instructions for Misconduct Reports

A referee must submit a separate misconduct report for each incident resulting in a player ejection to FWASA within 48 hours. The misconduct report must submitted by email to vp@fwasa.org AND office@fwasa.org. Registered referees can click here (Currently Unavailable) to submit misconduct reports online.

A clear, readable, and complete misconduct report is essential for the Appeals and Disciplinary Committee (A&D) to function effectively. Describe the event in enough detail to communicate the severity of the player’s misconduct. Keep in mind that the people reviewing your report did not witness the misconduct and are relying on your description of the event to sanction the player with the an appropriate fine and suspension.
