Instructions for Misconduct Reports

A referee must submit a separate misconduct report for each incident resulting in a player ejection to FWASA within 48 hours. The misconduct report must submitted by email to vp@fwasa.org AND office@fwasa.org

A clear, readable, and complete misconduct report is essential for the Appeals and Disciplinary Committee (A&D) to function effectively. Describe the event in enough detail to communicate the severity of the player’s misconduct. Keep in mind that the people reviewing your report did not witness the misconduct and are relying on your description of the event to sanction the player with the an appropriate fine and suspension.

Referee Pay Requests

Forfeits:
FWASA will make every effort to give at least 24 hours notice for a forfeited game. In the event that notification does not meet that timeframe, FWASA will pay referees for the game. Referees must fill out and submit a pay request to the FWASA office for each missed game. All reports must be submitted within 30 calendar days of the game--no exceptions.

Cancellations Due to Weather/Unforeseen Events:
If a game is cancelled due to weather-related closures and other events outside of FWASA's control, please check with Lalo on whether payment will be issued.
