NEW! Player/Team/Referee Complaint Form

Players and Managers,

As of our most recent board meeting, the FWASA board decided to create and implement a new form to more accurately keep records of complaints raised by players or teams about another player, team, or referee.  We recognized that teams could provide feedback on referees via game reports and by requesting one referee not work their games per season, and referees can request to not work with a particular team by coordinating with the referee assignor. However, we didn't have any official way to log issues that came up during games but were unnoticed by referees.  We wanted to create a way to formally log and track any of these type of issues that arise.

Therefore, we've created the FWASA complaint form.  This form is posted on the FWASA website, and should be filled out completely in order for the A&D committee to place the complaint form in the file regarding a particular player or team.  You will find the Download link to the right.

As noted on the form, the A&D committee will not take disciplinary action or dispense punishments solely based on these reports. However, the reports could be considered as additional information should the A&D committee need to respond to a punishable offense (like a red card).  The A&D committee always reviews player/team history when determining punishments, so this would not be a new process, but just an added piece of information.

For example, let's say that during a game one of our teams was the victim of racist comments from an opposing team member. Unfortunately, I have heard reports of this behavior and it is completely unacceptable.  After the game, any FWASA player or manager can fill out the form, and the A&D committee will keep record of this particular player's behavior.  Please type the forms if at all possible to avoid and issues with reading your handwriting!  The forms should be submitted online or sent via email to office@fwasa.org and vp@fwasa.org, as well as your league commissioner.  If the player is red carded for racist comments or verbal abuse in a FWASA game later in the season, the A&D committee could take the FWASA complaint form into consideration when determining punishment.  Any actions taken as a result of these forms being filed are completely up to the discretion of the A&D committee.

We hope this form will not have to be used often; but when they are necessary, we hope to be able to gather better information in order to make better decisions, and ultimately to make FWASA the best place to play soccer around!
